Custom Care provides quality home care services to people living across the UK. We enable our service users to access the expert care they need, whilst continuing to live in the homes that they know and love. An opportunity has now arisen to join our team as a Care Coordinator at our Custom Care branch in Solihull (B92 7HX). The role of the care coordinator is to organise the effective delivery of high-quality care services in specified localities and/or under specified service contracts. The care coordinator is expected and authorised to: Organise and coordinate care services, with the safety and welfare of service users in staff foremost in their consideration; Actively work to increase the volume and quality of care support services; Line manage care workers and other designated juniors; Maintain clear, accurate, appropriate and up-to-date records, whether electronic or in other formats; Have due regard for the health and safety of themselves and others; Practice and promote effective, timely communication both within and outside the company; Protect confidential information in line with the group’s information governance policy; Participate in staff development (including supervision), training and performance appraisals as required; Participate in any meetings as required; Provide management information as required The Care coordinator will: Accept and implement referrals for care services from commissioning organisations; Contribute as required to needs and risk assessments in respect of individual service users; Work with recruitment personnel to ensure sufficient current and future staffing levels; Ensure that care worker rotas are organised and sufficiently well-planned, taking into account travelling time; Plan and allocate care assignments to appropriate care workers, with due regard for the particular needs of the individual and the particular skills and attributes of the worker; Liaise as necessary with partner professionals and organisations (e.g. social workers, GPS, nurses) in respect of developing and delivering holistic, person-centred packages of care; Engage and work closely with unpaid carers, advocates etc. for the benefit of the service user; Encourage and assist service users to achieve optimum independence, with due regard for their privacy, confidentiality, dignity, health, safety, welfare, living environment, personal choice, lifestyle, nutrition, hydration, independence, self-esteem, physical comfort and social and emotional needs; Ensure that care support is delivered in a manner which is sensitive to the service user’s culture, disability, race, age, sexuality, marital or civil partnership status, transgender status or religion/belief; Act promptly and appropriately to protect service users from neglect or abuse; Monitor service user's condition and circumstances and act appropriately when these change; Participate as required in the multi-disciplinary review of services; Receive and handle complaints about services from service users or others; Contribute as required to administrative processes such as payroll and invoicing; Carry out on-call coordinating duties outside normal office hours as agreed. All care coordinators will be subject to an enhanced level criminal records Disclosure. All care coordinators are required to register as Dignity Champions.
