We are looking for an energised and passionate Care Coordinator like you (known internally by us as a Supported Living Care Coordinator – Level 2) to join our service which provides support to clients with autism and learning difficulties. As Lead Care Support Worker you’ll coordinate the work of our inspirational team of Support Workers, leading the shift and supporting your colleagues to ensure that person centred care is delivered to our clients. Together you will really make a difference, empowering our clients to achieve their goals and aspirations as outlined in their support plans and develop the independent living skills they need to successfully move on with a positive future. Managing a small caseload of clients, you’ll oversee personal care that is delivered to our clients and be key in ensuring that CQC, care regulatory and supported living quality standards are achieved at all times. You’ll also lead on planning and delivering a weekly programme of activities for our clients and will undertake risk assessments, care planning and regular reviews too. It’s really important to us that you have time to what you are really passionate about; making a real difference to our clients lives. So to free up your time, you’ll be given the latest IT kit, which could include a Surface Pro tablet and smart phone, supported by our new online integrated IT system which means that you don’t have to be in the office as you can complete support plans on the go. With previous experience of working with vulnerable people providing care and support, you will have a great understanding of professional boundaries, safeguarding, risk assessment, confidentiality and be experienced in case management. It’s also important that you can work in holistic way building partnerships with external agencies and that you have strong IT skills too. The working hours will be weekend based, Saturday 8am-4pm and Sunday 3:30pm-9pm If you are successful, we need you to have an Enhanced Disclosure Check through the Disclosure and Barring Service which we’ll pay for.
