The Post This post is within the Older People and Physical Disabilities Support Planning and review Team and will be responsible for assessments, Support Planning and reviewing for one or more of the following client groups: Adults with Physical disabilities, Older people, Older Peoples Mental Health. We are looking for people who can build positive relationships at a number of levels and can bring enthusiasm, creativity and passion to the service. You will need to be passionate about serving our customers, have excellent communication skills and the creativity to find solutions that deliver excellent services our customers deserve. This post is responsible for assessing the needs of vulnerable adults and their carers. The job will require you to promote service user independence and produce robust outcome focused support plans; identify, manage and support positive risk taking and will require working in partnership with other statutory and non-statutory agencies to provide seamless service. The Person Have you got the relevant professional qualification in Health or Social Care? Do you have knowledge and experience of the needs of older people, Older peoples Mental Health, adults with physical disabilities? Are you able to work positively as a member of the multi-agency team on complex and multi-disciplinary cases involving mental health, voluntary organisations and NHS/CCG partners? Are you able to put together Care packages and Support Plans including monitoring and reviewing? Are you able to purchase Care Packages within Budgetary Framework?
