JOB TITLE: Assessment and Pathway Coordinators– (Homeless) 

SALARY & BENEFITS: £18,431 starting + full training & car allowance

WORKING HOURS: 37 hours a week- full time –flexibility required on occasions 

THE COMPANY: Our client, based in Bedfordshire, is a value-based homeless charity organisation, with facilities in Luton & Dunstable. They are committed to putting the people they support in control of their lives. They offer temporary accommodation with support services & focus on enablement and maximising independence to help clients achieve their own potential to live and develop in the community 

THE ROLE: The role of the Assessment and Pathway Coordinator is to maintain a centralized point to receive and assess all referrals into Support Services ensuring all Signposts accommodation projects operate at a minimum of 90% occupancy 

Monitor client’s progress against their support plan undertaking quarterly support plan reviews, challenging clients and key workers to always raise expectations

Key responsibilities: - 

Take responsibility for assessing and allocating keyworkers to clients referred to Signposts (Luton) using an asset-based personalisation process 
Provide on-going pathway co-ordination in collaboration with the Signposts Keywork Support Team, statutory and specialist agencies, ensuring clients are supported to meet their aspirations in full, become fully socially included and are involved in decision making 

Using a variety of resources gather as broad a picture of the client and their history as possible to enable correct allocation

Attend regular meetings with the Assessment and Pathway Co-ordination and Keywork Support Teams to identify, progress and resolve issues

Key experience for this role: - 

Previous experience in a similar/ caseload management, people facing role from the Supported Housing, Health & Social Care, employability or another similar service sector would be a distinct advantage
Experience in a role helping and dealing with people with challenging behaviours is desirable but not essential ( Full training will be given)
Computer literate
Hold a full UK driving licence and access to transport is essential. ( Car Allowance provided-occasional driving of clients will be required)
Empathy, caring and sensitivity to the needs of others


Previous experience working as Home Carer, Care Worker, Health Care Assistant, Support Worker, Nurses, Social Care Worker, Care Assistant, Mental Health Care Worker, Employment Advisor, Voluntary Worker are all desirable but not essential as full training will be provided.
