JOB TITLE: Care Co-ordinator (Community)

SALARY & BENEFITS: £9.22 per hour + enhancements and on call payment

WORKING HOURS: Full time 32.5 hours a week (Mon-Fri)- Perm contracts

THE COMPANY: Our client is the UK’s leading charity supporting disabled people. For over 65 years they have supported disabled people in many different ways, to reflect their needs including providing care in people’s own homes, through to employment and education.

THE ROLE: Care Co-ordinator (Community), Sheffield

Reporting to the Service Manager the role is based at the Sheffield Support at Home office at Mickley Hall which provides care and support to both disabled and older people living in their own homes within the Sheffield Area.
      You will support people with all aspects of daily living, ensuring that their independence is preserved and developed.

Key Responsibilities to include:- Care Co-ordinator (Community), Sheffield
  • Computerised preparation and distribution of support worker rotas and client Schedules.
  • Undertaking the initial assessments for care and support and health & safety.
  • Preparation of Individual support plans and monitoring outcomes are being met in line with current legislation, policies and procedures.
  • Liaise with support staff on a regular basis and conduct individual supervisions and support to ensure contractual obligations are met.
  • Assist in recruitment.
  • Participation in the on-call/out of hour’s service on a rota basis including evenings and weekends is required. (Additional payment is made for this)

Key Experience preferred for the role:- Care Co-ordinator (Community), Sheffield
  • Previous experience in a Care Co-ordinator/Senior/Supervision or Team leader role from a Social care background essential
  • Experience of doing work rotas, client assessments & client schedules Essential
  • NVQ 2 in Care/ Diploma in social care or Level 3 or willing to work toward qualifications
  • Ability to maintain accurate records and have good written and verbal communication skills.
  • A full driving license you can use in the UK
  • Ability to work on call rota hours when required, and to travel to fulfil the objectives of the job essential
